Pre-injection checklist

  • Confirm the medication is subcutaneous, not intramuscular. GLP-1 medicines (Ozempic, Wegovy, Mounjaro, Zepbound) and most insulin are subcutaneous and use a shallow angle and a short needle.
  • Check your needle length. A short 4 mm to 6 mm pen needle is designed for a 90-degree insertion in most adults; a longer needle or a very lean site may call for 45 degrees or a skin pinch.
  • Pick a site with a layer of fat just under the skin: the abdomen at least two inches from the navel, the front of the thigh, or the back of the upper arm.

    Default to 90 degrees with a short needle

    For a 4 mm to 6 mm pen needle in most adults, insert straight in at a 90-degree angle (perpendicular to the skin). Short needles are built to reach the fat layer at 90 degrees without entering muscle, which is why the GLP-1 and insulin pen-needle guidance standardizes on them.

    Use 45 degrees only when advised

    Switch to a 45-degree angle when you are using a longer needle (8 mm or more) or injecting a very lean site with little fat, where 90 degrees could reach muscle. A shallower angle keeps the tip in the subcutaneous layer. This is a clinician's call, not a default.

    Pinch the skin to set the depth

    Lift a fold of skin and fat between thumb and forefinger when the site is lean or the needle is on the longer side. The pinch raises the fat away from the muscle so the needle stays subcutaneous. For a short needle in an area with ample fat, a flat stretch is usually enough.

    Insert in one smooth motion

    Hold the chosen angle steady and push the needle in with a single, confident movement. A slow, hesitant entry stings more and can skim the skin instead of seating cleanly in the fat.

    Deliver, hold, and withdraw at the same angle

    Press the plunger or dose button fully, hold for the count the device specifies (often several seconds), then withdraw the needle along the exact angle you entered. Changing angle on the way out drags the needle and bruises the tissue.

After the injection

  • Press a clean tissue or cotton ball to the site for a few seconds; do not rub.
  • Drop the used needle straight into the sharps container without recapping.
  • Rotate to a different spot next time so no single site is overused.

Common mistakes

The aim is consistency: the same subcutaneous depth every time gives steadier absorption. The angle matters mainly because it controls depth. A short needle at 90 degrees and a longer needle at 45 degrees can both land in the same fat layer. Mixing depths from dose to dose is what causes uneven absorption, not the number of degrees itself.

Is the angle different for the abdomen, thigh, and arm?+

The principle is the same at every site: reach the fat just under the skin and avoid muscle. The abdomen usually has the most fat and tolerates a flat stretch at 90 degrees well. Lean thighs or arms may need a pinch or a 45-degree angle. Choose the angle by the fat you can feel at that site, not by the body part alone.
